Massage Therapy is available at LifeForce Chiropractic! This may be a covered benefit with your insurance company.

To determine if this is true for you, please follow these steps.

Call your insurance representative to pre-qualify this service. The customer services or member services number is located on your insurance card.
Ask if your insurance will pay for procedure code:
97124 (Massage) Yes / No
97140 (Myofascial Release/Trigger Point Therapy) Yes /No
Be sure to mention that this procedure is to be received at a Chiropractic Clinic, not a massage therapy clinic.
Ask if a referral is necessary. Yes / No
If a referral is necessary:
Will a referral from a Chiropractor be acceptable? Yes / No
Or will a referral from a Physician be required? Yes / No
If this is a covered benefit, I would suggest that you record the name of the representative that you spoke to, as well as the date and time of your call. This will be helpful in the case of future difficulty in collecting for reimbursement.

If you choose to take advantage of this benefit, you will be billed at the time of service rendered. We will provide for you a super bill (receipt) that has the information necessary to submit to your insurance company. Your insurance company will then reimburse you directly.

The charges for massage are as follows:
1/2 hour $40
1 hour $60
1.5 hours $90
